Responding to pressure in the healthcare system to create local communities of care, North York General Hospital (NYGH) in Toronto is working with partners across primary care, long-term care, home care, mental health and community support services to build collective capacity and create a Health Team to meet community needs.
NYGH adopted a data-driven approach to understand and describe determinants of health in their catchment using analytics to reveal relationships between hospital patients and local community demographics. Using mapping to visualize hotspots, they can look at disease prevalence distribution such as COPD, asthma or diabetes and link it with patients diagnosed with these conditions. For a complete patient-centered approach to care, they are building neighborhood snapshots and linking socioeconomic factors to the health of communities of populations.
With the help of IBM Cognos Analytics, NYGH has developed a variety of analytics solutions that enable decision makers to visualize and access the community data and understand gaps in services. This helps promote innovative planning and strategy to meet community needs.
